| Annual Permit Category | Max Width | Max Height | Max Length | Max Weight |
|---|---|---|---|---|
| Combination or Equipment | 12' | 14' | 75' | 150,000 lbs (routing authorization required over 100k) |

| Emergency Tow | 12' | 14'6" | 135' | 160,000 lbs |
Alabama's $100 flat annual is among the cheapest in the country — the same fee covers all six categories above. The Emergency Tow permit allows the highest weight (160,000 lbs) and longest length (135'). Loads over 100,000 lbs on the Combination/Equipment permit require a separate routing authorization.

Arkansas has no general annual OW permit — only these specialized categories. The $800 agronomic/horticultural fee (Rule 19) and $250/zone forestry fee (Rule 21) are set by ARDOT rules. The wrecker/tow and 57' trailer fees are set at $500 per the rules. Animal feed and farm equipment dealer fees are "up to $500" per statute.

Connecticut uses a weight-based formula ($9 per 1,000 lbs) rather than flat fees. The $12 transmittal fee increased from $5 in July 2022 per Public Act 22-40. Annual indivisible permits are restricted to CT-based carriers, and bulk milk permits run May 1 — Apr 30 with prorating available.
Delaware does NOT issue annual oversize/overweight permits. All oversize/overweight moves require individual single-trip permits.
| Permit Type | Fee | Details | Duration |

| Single trip (minimum) | $30 | Base fee for OS and/or OW | 5 days |
| OW surcharge | $10/8,000 lbs over legal | Added to base fee for overweight loads | per trip |
| Superload | $60 minimum | GVW over 120k, or L >= 120', or W >= 15', or H >= 15' | 5 days + up to 14-day bridge review |
Delaware is one of the most restrictive states for repeat heavy haul — no annual option means every move requires an individual permit. Movement is restricted sunrise to sunset Monday through Sunday unless special provisions apply.

Florida has the most granular annual system of any state — 8 OW tiers from $240 to $500, plus separate overdimensional tiers. The $500 tier covers up to 199k GVW; above that requires individual superload review and no annual is available. FL also offers 3-month route-specific permits at approximately 25% of annual cost. Current fee schedule effective 04/24/2018 per FAC 14-26.008.

Idaho's annual OW permit ($128) is a base fee — the real cost comes from quarterly per-mile road use fee reports based on laden mileage at the permitted weight level. Annual permits may not be issued for gross weights exceeding 200,000 lbs. The 129,000 lb interstate exemption under 23 USC 127 applies.
Illinois calls its annual/multi-trip permits "Limited Continuous Operation" (LCO) permits. They are available in 3-month and 12-month durations. Fees vary by load type, dimensions, and whether oversize-only or oversize+overweight.

| 110,001 - 120,000 lbs | $150/month | $115/month |
Overweight — Class A Special Mobile Equipment:
| Permit Type | Fee | Duration |
|---|---|---|
| Class A SME overweight | $25/month | 1-12 months |
Combined OW + OD: $25/month OD fee added on top of the overweight fee.
Additional Fees
Additional fees: Maine Turnpike requires separate $10 permit for OS/OL moves (per trip, not annual).
Long-term permits are vehicle-specific, issued monthly for 1-12 months. At full 12-month term: OD-only = $300/yr; 6-axle OW at 110k = $1,080/yr; 6-axle OW at 120k = $1,800/yr; 7-axle OW at 110k = $780/yr; 7-axle OW at 120k = $1,380/yr; Class A SME = $300/yr. The 7+ axle configuration is cheaper than 6-axle because the weight is better distributed.

Michigan calls these "extended transport permits" rather than "annual permits." Permits expire on a staggered schedule based on the first letter of the permittee's last name. The $100 OW annual is among the cheapest in the country for general OW. Max dimensions on extended permit: W 16' (14' during spring thaw), L 150', H 15'.

The $60 ODV Annual is the primary annual permit, primarily for oversize (over-dimension), with a 20,000 lb single axle cap under NRS 484D.735. Annual permits are truck-specific, not commodity-specific, and do not require a route.
Annual Permits
| Permit Type | Fee | Duration |
|---|---|---|
| Statewide annual | $115 | 12 months |
| 100-mile radius annual (NH-based carriers only) | $60 | 12 months |

The $60 local permit is specifically for a 100-mile radius and is only available to NH-based and NH-registered carriers. The $115 statewide annual is available to all. Annual permits do NOT cover overweight — they are for legal weight only. Any non-divisible load exceeding legal weight on an annual permit must apply for a supplemental permit. NH Turnpike has separate permitting.

NJ does not offer general annual OS/OW permits. The Code 23 system is a registration + per-trip route permit structure, not a true annual blanket permit. The $209.50 is a vehicle registration fee, not a permit fee. The only true annual multi-trip permit is the ocean-borne container permit at $117.60.

ND does not appear to offer a general annual overweight permit for non-divisible loads — only the 129k divisible load annual and the OS-only annuals (overwidth/overlength). The annual overwidth ($150) and overlength ($150) permits allow carriers to avoid individual trip permits for loads within those dimension limits.

Ohio's continuing annual permits are among the priciest in the country, reflecting the state's toll-like structure for heavy haul. The "Operation Open Roads" blanket permit program for loads up to 90,000 lbs without pre-approval has been continued beyond its pandemic-era origins. Loads that are both OS and OW pay the combined OS/OW rate, not separate fees.

Additional fees: Road Use Assessment Fee (RUAF) applies to single-trip non-divisible loads (ORS 818.225). COVP agent fee reduction of $2.75 per jurisdiction when issued by contracted agent.
Oregon's primary annual permit is the COVP — $3 to ODOT plus $8 per local jurisdiction included on the permit (reduced to $5.25/jurisdiction when agent-issued). Per ORS 818.270, the maximum fee is $8 per road authority plus up to $5 additional for private contractor issuance. Extended weight and heavy haul annual permits are available but fees are determined through ODOT's ORION permitting system rather than being flat statutory rates.

South Dakota does not offer a general annual OW permit for non-divisible freight. Instead, SD uses an extensive Extended Period permit system — most categories are $60 flat. The Multiple-Trip Construction Equipment permit is uniquely cheap at $5/county/month (max $20/month) but is not valid on interstate. The LCV permit covers up to 129,000 lbs but is sold in books of 10 single trips, not as a true annual.

The OW annual ranges from $750 (up to 100k) to $3,500 (up to 165k), while each OD annual is a flat $100. Tennessee also offers extensive specialty annual permits for mobile homes, tow trucks, ocean containers, and non-commercial boats.

Texas has one of the most extensive annual permit systems in the country with 20+ categories. The $4,000 envelope permit (12' wide, 14' high, 110' long, 120,000 lbs max) is the primary general-purpose annual. The Over Axle/Over Gross Weight Tolerance permit scales by county count from $270 (1--5 counties) to $1,095 (statewide). TX also offers 30/60/90-day width/length permits at $120/$180/$240.

Utah uses a 3-tier GVW structure for both annual and semiannual permits. The statute distinguishes between nondivisible (up to 125k) and divisible load (up to 129k) permits. Semiannual runs 75--78% of annual cost, making the annual the better value for year-round operations.

Vermont offers both oversize blanket permits and special excess weight permits as annual options. Permits expire with vehicle registration (out-of-state vehicles expire Dec 31). The special excess weight permits are annual overweight permits for divisible loads at specific weight tiers — upgrades to heavier weights require paying only the fee difference.

Divisible load multi-trip permits are not valid during spring thaw except where specified, and permits are not valid on posted bridges.
Extended Period Permits (No Annual Permit Available)
Wyoming does NOT offer an annual oversize/overweight permit. The longest multi-trip option is 90 days.

| Overall combination | 120' |
| Single axle | 25,000 lbs |
Wyoming uses a permit class system (A through W). Class D is the closest to an "annual" permit — it's an extended period permit for a specified vehicle on specified routes, but duration is not capped at 90 days like E/F. Class E (oversize) and Class F (overweight) are commodity-specific and capped at 90 days. Class F permits are valid on primary and secondary highways only — no interstate. All overweight fees include a $0.06/ton-mile component based on excess weight over statutory limits.
